Completed forms must be turned in to the Athletic Office before you can participate in any sport.  You must also go to the athletic office and pick up the OHSAA Athletic Eligibility Bulletin.  You are required to read this bulletin.  This form does not need to be returned.  Your signatures on the Athletic Participation Form acknowledges you read it.

We would appreciate your help in completing the necessary forms for your athlete to be eligible to participate in an inter-scholastic athletic program sponsored by the Plain Local School District.  The forms needed are:

1. Physical Exam Form:
A doctor's physical examination is necessary to verify that your athlete is physically able to participate in sports. If you are aware of special medical concerns of your athlete, we ask that you make the coach and athletic trainer aware of the problem. 2. Emergency Medical Authorization Form:
This form provides updated information to our coach, team trainer and team doctor in case of emergency. This form is taken to all away trips and if an accident or injury occurs, and if medical treatment is necessary, your signature provides approval to administer treatment. 3. Athletic Participation Form:
This form provides residency information, an insurance waiver, information on returning athletic equipment, and permission for transportation and athletes being photographed. Both the athlete and parent/guardian must sign this form. 5. Pay to Participate Form: UPDATED

6. Drug Testing Consent: UPDATED

7. Athletic Department Travel Release:
This form allows students to travel to and from Athletic events.
