In dramatic fashion, the Canal Winchester Football Program recorded it's first Football post season win. Down by 2 points with 3.4 seconds remaining in the game, Tyler Shryock was called upon to attempt a 21 yard Field Goal, and with the poise and confidence of a seasoned veteran, the sophomore split the uprights as time expired to deliver the biggest kick of his career so far. The Field Goal made the final score 17-16 and earned the Indians a trip to the DIII Regional Semifinals next week against Monroe High School. Scoring: Hitchens 65 fumble return for Touchdown. Davis 1 yard Touchdown run. Tyler Shryock 2 extra points and a game winning Field Goal.

Canal Winchester Student Athlete Resources

All completed Athletic forms must be turned in to the Athletic Office before you can participate in any sport.  Go to the Student Athlete resources page for more information.

You must also go to the athletic office and pick up the OHSAA Athletic Eligibility Bulletin.  You are required to read this bulletin.  This form does not need to be returned.  Your signatures on the Athletic Participation Form acknowledges you read it.

We would appreciate your help in completing the necessary forms for your athlete to be eligible to participate in an inter-scholastic athletic program sponsored by the Plain Local School District.  The forms needed are:

1. Physical Exam Form:
A doctor's physical examination is necessary to verify that your athlete is physically able to participate in sports. If you are aware of special medical concerns of your athlete, we ask that you make the coach and athletic trainer aware of the problem. This is very important for us in handling treatment of an injury or medical problem. (two-sided form).  Click here to open and print the 2006 - 2007 Physical Exam form.

2. Emergency Medical Authorization Form:
This form provides updated information to our coach, team trainer and team doctor in case of emergency. This form is taken to all away trips and if an accident or injury occurs, and if medical treatment is necessary, your signature provides approval to administer treatment. 3. Athletic Participation Form:
This form provides residency information, an insurance waiver, information on returning athletic equipment, and permission for transportation and athletes being photographed. Both the athlete and parent/guardian must sign this form. 5. Pay to Participate Form:
This form includes information on fees and due dates for all Student Athletes.  Click here to open a printable Pay to Participate form.

6. Drug Testing Consent:
This form allows Coaches and qualified personel to drug test all student athletes. Click here to open a printable Athletic Drug Testing Consent form.

7. Athletic Department Travel Release:
This form allows students to travel to and from Athletic events.

These forms MUST be completed and turned in to the coach BEFORE your athlete is eligible to practice.  Please get these forms completed as soon as possible so that your athlete is able to begin practice at the proper starting date.  If you have any questions, please contact the Athletic Office at 920.2641
