In dramatic fashion, the Canal Winchester
Football Program recorded it's first Football post season
win. Down by 2 points with 3.4 seconds remaining in
the game, Tyler Shryock was called upon to attempt a
21 yard Field Goal, and with the poise and confidence
of a seasoned veteran, the sophomore split the uprights
as time expired to deliver the biggest kick of his career
so far. The Field Goal made the final score 17-16 and
earned the Indians a trip to the DIII Regional Semifinals
next week against Monroe High School. Scoring: Hitchens
65 fumble return for Touchdown. Davis 1 yard Touchdown
run. Tyler Shryock 2 extra points and a game winning
Field Goal.

Tony Davis ran for 102 yards and a Touchdown as the Indians
recorded their third shutout of the season.
Scoring: Romanowski 5 yard TD from Cosby pass.

Davis 3 yard TD run, 3 yard TD run, 11 yard TD run. Romowski
7 yard TD pass from Stevenson. Cosby 21 run. Shryock 5 extra
points.
September 28, 2007 - MSL Canal Winchester 12 Fairfield Union 14
Tony Davis rushed for 192 yards and had Touchdown runs of
89 yards and 70 yards as the Indians were handed their first
defeat of the season. They are now 5-1 overall and 2-1 in
the MSL.

Athletic forms must be turned in to the Athletic Office before
you can participate in any sport. Go to the Student
Athlete resources page for more information.
You must also go to the athletic office and pick up the OHSAA
Athletic Eligibility Bulletin. You are required to read
this bulletin. This form does not need to be returned.
Your signatures on the Athletic Participation Form acknowledges
you read it.
We would appreciate your help in completing the necessary
forms for your athlete to be eligible to participate in an
inter-scholastic athletic program sponsored by the Plain Local
School District. The forms needed are:
1. Physical Exam Form:
A doctor's physical examination is necessary to verify that
your athlete is physically able to participate in sports. If
you are aware of special medical concerns of your athlete, we
ask that you make the coach and athletic trainer aware of the
problem. This is very important for us in handling treatment
of an injury or medical problem. (two-sided form). Click

2. Emergency Medical Authorization Form:
This form provides updated information to our coach, team
trainer and team doctor in case of emergency. This form is
taken to all away trips and if an accident or injury occurs,
and if medical treatment is necessary, your signature provides
approval to administer treatment. Please print and fill out
two copies. Click here to open a printable Emergency Medical Authorization
form.
3. Athletic Participation Form:
This form provides residency information, an insurance waiver,
information on returning athletic equipment, and permission
for transportation and athletes being photographed. Both the
athlete and parent/guardian must sign this form. (Since the
OHSAA Eligibility Bulletin in now a part of the Acknowledgement,
it is no longer necessary to sign and return this form).

These forms MUST be completed and turned in to the
coach BEFORE your athlete is eligible to practice. Please
get these forms completed as soon as possible so that your
athlete is able to begin practice at the proper starting date.
