Canal Winchester Student Athlete Resources

All completed Athletic forms must be turned in to the Athletic Office before you can participate in any sport.

You must also go to the athletic office and pick up the OHSAA Athletic Eligibility Bulletin. You are required to read this bulletin. This form does not need to be returned. Your signatures on the Athletic Participation Form acknowledges you read it.

We would appreciate your help in completing the necessary forms for your athlete to be eligible to participate in an inter-scholastic athletic program sponsored by the Canal Winchester Local School District.

These forms MUST be completed and turned in to the coach BEFORE your athlete is eligible to practice. Please get these forms completed as soon as possible so that your athlete is able to begin practice at the proper starting date. If you have any questions, please contact the Athletic Office at 920.2641
